 Manguito rotador es un grupo de tendones que unen el brazo con cuatro músculos del hombro. El hombro se mueve con mayor libertad y en más direcciones que cualquier otra articulación. El hombro es una articulación esférica de poca profundidad, -enartrosis-, lo que la hace la articulación más dislocada del cuerpo. Sujeto a muchas enfermedades crónicas, el manguito rotador puede mantenerse saludable y fuerte, practicando estiramientos y ejercicios.

Abstract

PURPOSE:

The purpose of this study was to define the subscapularis tendon footprint anatomy in 3-dimensional (3D) perspective, report the incidence of tears, and classify the tear patterns prospectively during shoulder arthroscopic surgery.

CONCLUSIONS:

The first facet of the subscapularis tendon footprint consists of approximately one third of the entire footprint, and the first 2 facets consist of 60% of the entire footprint. The probability of finding any extension of the subscapularis tendon tear occurs in approximately 50% of the patients who undergo shoulder arthroscopy for all forms of shoulder disease. Among those subscapularis tendon tears, less than or equal to 80% are first facet tears.

Resumen
PROPÓSITO:

El propósito de este estudio fue definir la huella anatómica del tendón subescapular en una perspectiva en 3 dimensiones  (3D), el informe de la incidencia de los desgarres, y clasificar los patrones de los desgarres prospectivamente durante la cirugía artroscópica de hombro.

CONCLUSIONES:

La primera faceta de la huella del tendón subescapular consiste en aproximadamente un tercio de toda la huella, y las 2 primeras facetas consiste en 60% de toda la huella. La probabilidad de encontrar cualquier extensión de la rotura del tendón subescapular se produce en aproximadamente el 50% de los pacientes que se someten a una artroscopia de hombro para todas las formas de la enfermedad hombro. Test de Tensión en Miembro Superior – Un estudio cadavérico

Las pruebas de tensión neural de los miembros superiores (ULNTT en inglés) se utilizan habitualmente en el cuello y los miembros superiores para diferenciar patologías nerviosas de otras patologías musculoesqueléticas. El presente estudio pretende identificar el efecto del ULNTT del nervio mediano en el desplazamiento de la raíces nerviosas cervicales de C5 a C8 en cadáveres.

Los autores encontraron que la prueba mostró un desplazamiento inferolateral significativo de las raíces nerviosas.

La Prueba de Tensión Neural de Miembro Superior 1 (ULNTT1) para el nervio mediano se realizó en 11 cadáveres, 6 hombres, 5 mujeres, con una edad media de 80 años. La prueba se realizó con los siguientes movimientos pasivos: depresión del hombro, abducción de 110 grados, rotación externa del hombro máxima, extensión máxima del codo, extensión de muñeca y dedos. No se realizó flexión lateral del cuello. Marcadores radiotransparentes implantados en las raíces nerviosas cervicales C5-8 se utilizaron para identificar el desplazamiento y la tensión a través de imágenes fluoroscópicas. El desplazamiento fue en una dirección inferolateral (paralela con el eje de la raíz) y varió de 2.16mm a 4.32mm con las raíces nerviosas C6 y C7 muestran la más desplazamiento y C5 lo menos.

El estudio no mencionó en qué momento las raíces de los nervios comenzaron a desplazar debido a que las imágenes solamente se tomaron en un punto neutro y de nuevo al final del rango. Abstract

STUDY DESIGN:

A cross-sectional cadaveric examination of displacement and strain measured at the level of the cervical nerve roots during upper limb neural tension testing (ULNTT) with median nerve bias.

OBJECTIVE:

To determine the displacement and strain of cervical nerve roots C5-C8 during ULNTT with minimal disruption of surrounding tissues.

SUMMARY OF BACKGROUND DATA:

Clinical examination of neural pathology involving cervical nerve roots is difficult because of the transient nature of pathologies, such as cervical radiculopathy, entrapment neuropathies, and thoracic outlet syndrome. Cadaveric studies have demonstrated significant displacement and strain in lumbosacral nerve roots during neurodynamic testing of the lower extremity. Examination into the biomechanical behaviors of cervical nerve roots during ULNTT has not been performed.

METHODS:

Eleven unembalmed cadavers were positioned supine as though undergoing ULNTT. Radiolucent markers were implanted into cervical nerve roots C5-C8. Posteroanterior fluoroscopic images were captured at resting and ULNTT positioning. Images were digitized and displacement and strain were calculated.

RESULTS:

ULNTT resulted in significant inferolateral displacement (average, 2.16 mm-4.32 mm, P < 0.001) of cervical nerve roots C5-C8. There was a significant difference in inferolateral displacement between the C5 and C6 nerve roots (3.15 mm vs. 4.32 mm, P = 0.009). ULNTT resulted in significant strain (average, 6.80%-11.87%, P < 0.001) of cervical nerve roots C5-C8. There was a significant difference in strain between the C5 and C6 nerve roots (6.60% vs. 11.87%, P = 0.03).

CONCLUSION:

ULNTT caused significant inferolateral displacement and strain in cervical nerve roots C5-C8. These results provide the mechanical foundation for the use of ULNTT in clinical evaluation of pathology in the cervical region, such as in cervical radiculopathy, entrapment neuropathies, and thoracic outlet syndrome.

Abstract

PURPOSE:

The purpose of this study was to collect detailed data on postoperative sporting activity after arthroscopic Bankart repair for chronic shoulder instability.

CONCLUSIONS:

Arthroscopic Bankart repair provides a high rate of return to activity among patients treated for chronic shoulder instability. A number of previously inactive patients returned to activity postoperatively. However, one-third of patients reported no benefit from surgery in terms of sporting activity. The improvement in sporting proficiency was highly dependent on the demands on the shoulder in sports, as well as the age of the patient. Overall, there was no significant increase in duration, frequency, number of disciplines, Tegner activity scale, or performance level between preoperative and follow-up evaluation and no increased return to high-risk activities.

PROPÓSITO:

El propósito de este estudio fue recopilar datos detallados sobre la actividad deportiva postoperatorio después de una reparación artroscópica de Bankartpara la inestabilidad del hombro crónica.

CONCLUSIONES:

La reparación artroscópica de Bankart ofrece una alta tasa de retorno a la actividad entre los pacientes tratados por la inestabilidad del hombro crónica. Abstract

BACKGROUND:

The optimal treatment for symptomatic, nontraumatic rotator cuff tear is unknown. The purpose of this trial was to compare the effectiveness of physiotherapy, acromioplasty, and rotator cuff repair for this condition. We hypothesized that rotator cuff repair yields superior results compared with other treatment modalities.

CONCLUSIONS:

There was no significant difference in clinical outcome between the three interventions at the two-year follow-up. The potential progression of the rotator cuff tear, especially in the non-repaired treatment groups, warrants further follow-up. On the basis of our findings, conservative treatment is a reasonable option for the primary initial treatment for isolated, symptomatic, nontraumatic, supraspinatus tears in older patients.

ANTECEDENTES:

El tratamiento óptimo para sintomática, no traumática desgarro del manguito rotador es desconocida. El propósito de este estudio fue comparar la eficacia de la fisioterapia, acromioplastia, y la reparación del manguito rotador para esta condición. La hipótesis de que los rendimientos de reparación del manguito rotador resultados superiores en comparación con otras modalidades de tratamiento.

CONCLUSIONES:

No hubo diferencias significativas en el resultado clínico entre las tres intervenciones en el de dos años de seguimiento. La progresión potencial del desgarro del manguito rotador, especialmente en los grupos no reparado tratamiento, garantiza el seguimiento ulterior.
